We maintain accurate assessments, case notes, care plans and progress reports, document any change in your circumstances, and escalate any concern about your safety, wellbeing or ability to live independently to management and your care team without delay.
Our community support workers provide community access, transport, daily living skills and social participation support across Sydney.
