with you

A clear, documented process from first contact through to ongoing review.

Our support workers follow a transparent and well-structured process that guides their work with you from the very first contact through to ongoing reviews. Initially, they take the time to understand your unique needs, goals, and preferences, ensuring that every step is tailored to you. 

A detailed plan is then developed collaboratively, outlining the support you require and how it will be provided. Throughout your journey, your support workers maintain open communication, regularly reviewing and adjusting the support plan to reflect any changes in your circumstances or goals. 

This clear, documented process ensures consistent, personalized care and ongoing support that is responsive to your evolving needs.

Assessment

We assess your individual needs and circumstances through home visits, consultations and ongoing communication — identifying your social, emotional, health, welfare and daily living support needs.

Care and support planning

We develop and implement an individualised care and support plan based on your assessed needs, preferences and personal goals, agreed with you and your family or carer before support begins.

Coordinated service delivery

We coordinate the delivery of your home and community-based supports so that every service matches your assessed needs and agreed support goals.

Monitoring and review

We monitor your progress and changing circumstances, evaluate how well your current supports are working, and recommend adjustments to your plan when something needs to change.

Records, reporting and safeguarding

We maintain accurate assessments, case notes, care plans and progress reports, document any change in your circumstances, and escalate any concern about your safety, wellbeing or ability to live independently to management and your care team without delay.

Supporting effective communication

Effective communication upholds the rights of people with disabilities to have choice and control and to make decisions about their own lives.
