COMMUNITY SUPPORT

Expert Community Support Workers

Our experienced community support workers walk beside NDIS participants across Sydney — at home, in the community and everywhere in between. From getting to appointments and building daily living skills, to joining social groups and reconnecting with the people and places you love, we tailor every hour of support to your goals.

Who we are

Support that starts with understanding you

Great community support does not begin with a service list — it begins with a conversation. Our support workers take the time to visit you at home, meet the people who matter to you, and understand what a good week actually looks like in your life.

We assess your social, emotional, health, welfare and daily living needs through home visits, consultations and ongoing conversation. From there we build a support plan that reflects your preferences, your routine and the goals in your NDIS plan — and we keep adjusting it as your circumstances change.

Every support worker on our team is qualified, police-checked, NDIS Worker Screening cleared and trained in person-centred practice. Most importantly, they are matched to you — for language, interests, culture and personality.

What our community support workers do

A qualified support worker does far more than turn up. Here is what our team takes care of.

Understand your needs

A qualified support worker does far more than turn up. Here is what our team takes care of.

Build your support plan

We develop and implement an individualised care and support plan based on your assessed needs, your preferences and your personal goals.

Listen to you and your family

We consult with you, your family and your carers to identify anything getting in the way of your wellbeing, independence and participation in everyday and community life.

Review and adjust

We monitor your progress and changing circumstances, evaluate whether your current supports are working, and recommend changes to your plan when they are not.

Coordinate your services

We coordinate the delivery of your home and community-based supports so services are delivered in line with your plan and your agreed goals.

Keep you connected

We support you to maintain social connections and take part in community activities that suit you — building independence, confidence and genuine social inclusion.

Explain your options

We give you and your family clear information and guidance about the home-care services, support arrangements and service options available to you.

Document everything properly

We maintain accurate assessments, case notes, care plans and progress reports, and record any change in your circumstances or support needs.

Look out for your safety

We identify any concerns about your safety, wellbeing or ability to live independently at home and raise them promptly with management and your care team.

Keep improving

We contribute to the ongoing review of our home-care programs by identifying changing needs, service gaps and opportunities to improve outcomes for the people we support.
