We give you and your family clear information and guidance about the home-care services, support arrangements and service options available to you.
We maintain accurate assessments, case notes, care plans and progress reports, and record any change in your circumstances or support needs.
We identify any concerns about your safety, wellbeing or ability to live independently at home and raise them promptly with management and your care team.
